Frames Issue!

Problem is, when I am playing a graphically demanding game, IE Fallout 4, under a medium load environment, I shoot down to about 30-45 FPS.
I feel as though with my specs I should maintain 60FPS under medium loads; only diminishing under High loads.

Maybe its just the Crimson drivers that AMD has that are the cause.

Bottom Line - I want to know why my frames are suffering. I want to know if there's a program for Win 7 that may diagnose a software issue on my end or a hardware issue.

look at your gpu load with gpu-z

also look if you see some inconsistensy in core and/or memmory clock
See if you max out the cpu in task manager and use cpu-z to make sure your cpu clock is doing fine. thats my best advice for now

Btw is it only fallout 4 and starcraft or is it poor peformance overall

Both; I run Fallout 4 on max settings. Fallout 4 runs at 60 FPS (or higher) on max settings, unless in a medium work load environment, such as a city; dropping drastically to around 25 frames. I just don't think that's normal..

Then there is starcraft 2 . In my opinion is the least demanding game out there; I run on High settings on a scale from (low - medium - high - ultra - extreme).-yup- While on the "high" settings, if there are about 40 units on the screen I might see frames drop down to 25ish.

Sorry, I shut it down @2 minutes due to my GPU temperatures getting to an uncomfortable level for me. I want my 390 to live as long as I do, Kappa.

Im having same problem with my r9 390.

First thing I noticed are the high temperatures and low fan speed (94°C @ a constant 16% fan rpm). This was a bug in AMD drivers for a short time, but I think it is fixed now (there was a hotfix in the 15.11.11 beta driver).
Because your GPU is getting this hot it is thermal throttling, so your games run poorly.
If it is not fixed within the driver you can force a custom fan curve with an program like MSI Afterburner, until AMD releases a proper fix for this.

Superb diagnosis. I can't believe I missed that. Looking at the fan speed makes it pretty obvious; I thought it was a proc issue.
