Name Change Thing

Basically, I've taken the step to change my name on twitch, so if it's possible I'd like to have my tokens/achievements transferred.
Username on Cohhilition webpage is still EuklionLoL
Old username on Twitch was EuklionLoL
New username on Twitch is Euklion

So I basically removed the "LoL" part and would love to get my tokens to my new Twitch username. What is the procedure?
